Dear Parents/Guardians,

The Girls Swim Team has earned the opportunity to participate in the State Tournament November 14-16th at the U of M Aquatic Center.

The school will offer fan buses for Thursday’s (November 15th) swim competition for all interested students, parents, and fans (minimum of 30 passengers).  Any K-6 students need to have parent(s) with them on the bus to ride. No 7th -12th siblings will be allowed as chaperones for a K-6 student. Parents/Guardians are encouraged to have their children utilize school sponsored transportation unless you are personally transporting your child or have made arrangements with another responsible adult to transport your child.  Students transporting students is not recommended.

Please be aware students will be checked in with supervisors periodically during the event.   Concessions are available onsite or students can bring their own food, however all containers will be checked prior to boarding the bus.  Any pop containers must be sealed or they will not be allowed on the bus. Students who utilize school transportation are not permitted to ride on any form of public or private transportation other than a Foley bus while in the Twin Cities.

Please review with your child, your expectations of him/her prior to signing the form below…It is your expectations that need to be communicated when you choose to allow your child this privilege.

By signing this form I give permission for my son/daughter to attend the State Swim Meet at the U of M Aquatic Center.  I agree that my son/daughter will be subject to all school rules and policies. If my son or daughter is not utilizing school transportation, my signature below indicates I am aware of, and accept any or all liability concerning my son/daughter.
